Direct Deposit/Electronic Funds Transfer


If you are currently receiving benefit payments by check, Aurora National Life Assurance Company can send your benefit payments directly to your financial institution by Direct Deposit or Electronic Funds Transfer (EFT).  

With Direct Deposit or EFT, there's no worry about your benefit payment check being lost, stolen or misplaced.  If you use either option, you will no longer have to make special trips to deposit your check. This will be especially convenient in times of inclement weather, ill-health, or when you are away from home.

About Electronic Funds Transfer (EFT)
Electronic Funds Transfer, or EFT, is an electronic deposit made by Aurora to your savings or checking account without the use of the postal service.  Your money is available at the opening of business on the day your benefit payment is due.  If you have an interest-bearing account, your money begins earning interest immediately.

All Aurora forms are in Adobe Acrobat format. You will need the Adobe Acrobat Reader version 3.0 or higher in order to print these forms. If you do not have the Adobe Acrobat Reader installed on your computer, you may download it from Adobe  free of cost. You will also preferably need a laser printer to print them, although most ink jet printers will work. These forms will not print out correctly on dot matrix printers and some ink jet printers.

What information do I need to supply to complete the form?

You, the Annuitant, or the person to whom benefits are payable, should complete all identifying information, including:

  • Contract Number (Beginning with "C2" and ending with "A")
  • Social Security Number
  • Payee's Full Name
  • Payee's Mailing Address or Home Address
  • Bank Name
  • Bank Address (Some banks use a centralized processing address; please verify with your bank)
  • Bank Phone Number
  • Bank Account Number (Must be in your name)
  • Type of Account (Checking or savings; please verify with your bank)
  • Bank Routing Number (For EFT only: must be 9 digits; please verify with your bank)
  • If you are not able to complete this form, a duly appointed representative (guardian, conservator or attorney-in-fact) may arrange for direct deposit or EFT of pension benefits for you. Your representative will need to complete this form, sign it as your authorized representative (including the appropriate designation) and attach a certified copy of any court-issued document granting him or her authority to act in such capacity.  Please send this form and any attachments to our mailing address shown above.

What are the eligibility requirements to enroll in EFT?

  • Benefit Payment Dates. You may use the EFT option if your benefit payment date is on the 1st, 28th, 29th, 30th or 31st of the month whether your payment mode (frequency) is now monthly, quarterly, semiannual or annual.  Direct Deposit only is available for all other benefit payment dates.
  • Payment Minimums and Frequency.  Payments made by EFT must be $50 or more and paid at least annually.  Important Note:  If your payment amount does not meet the required minimum, your payment frequency may be changed to the next less frequent mode (e.g. from monthly to quarterly, quarterly to semiannual or semiannual to annual).

What is Direct Deposit?

Direct Deposit means that a physical check will be mailed via the postal service from Aurora directly to your financial institution instead of your home or mailing address. 

What are the eligibility requirements for Direct Deposit?

  • Benefit Payment Dates. The Direct Deposit option is available for all benefit payment dates whether your payment mode (frequency) is now monthly, quarterly, semiannual, or annual.
  • Payment Minimums and Frequency. Payments made by Direct Deposit must be $500 or more and paid at least annually.  Important Note:  If your payment amount does not meet the required minimum, your payment frequency will be changed to the next less frequent mode (e.g. from monthly to quarterly, quarterly to semiannual or semiannual to annual).

What is the time required to process Your EFT or Direct Deposit request?
Please allow up to 60 days for us to process your request.  After this period, you can expect to receive your next scheduled benefit payment through Direct Deposit or EFT.  (Be sure to read "Payment Minimums and Frequency").
